Why testing matters

Why a TSH test alone can miss Hashimoto's

TSH shows how your thyroid is working, but not whether an autoimmune process is affecting it.
This video explains what else may be checked for Hashimoto's.

Sound familiar

What a Hashimoto's flare feels like

Hashimoto's symptoms can change over time. Some people go through periods when several symptoms become more noticeable at once. You may not experience all of them, and the pattern can look different from one person to another.

Sudden, heavy fatigue that sleep does not fix

Brain fog that is worse than usual

Joint or muscle aches

Anxiety, low mood, or feeling not like yourself

Bloating or constipation

Hair shedding, or hair falling out in the shower

Feeling cold when nobody else is

Weight that does not match what you are eating or doing

Patients most often name stress, an illness, a run of bad sleep, or something they ate, and it is not always obvious which one set a flare off. If this sounds familiar, you are not imagining it and it is not in your head. Some of these overlap with perimenopause, low iron, blood sugar problems and PCOS, now officially PMOS, which is one reason the workup here is wider than a thyroid panel on its own.

Hashimoto's

What Hashimoto's disease actually is

Hashimoto's is an autoimmune condition. Your immune system gets its signals crossed and starts attacking your thyroid, treating it like a threat. Over time that damage tends to slow the thyroid down, which is called hypothyroidism. It is the most common reason a thyroid becomes underactive in the United States, and it is far more common in women.

It is usually picked up with a blood test for thyroid antibodies. Those are called TPO and thyroglobulin antibodies, and they show up when your immune system is actively going after thyroid tissue. Here is the part that explains a lot of frustrating years. Antibodies can be raised long before your thyroid hormone levels ever look abnormal. A routine screen usually checks TSH and stops there. So it is possible to feel awful, get tested, be told everything is fine, and be telling the truth the whole time.

How we start

What your first visit for Hashimoto's looks like

Every patient starts the same way. What changes is where your history and your results take the plan.

01

A 75-minute, in-depth first visit

Your doctor takes a full medical history: your symptoms, past labs, medications, family history, diet, sleep, stress and hormone health. We also ask what you think is going on.

02

The testing your case calls for

Antibodies are part of the workup here rather than an extra you have to know to ask for. Draws are done locally through LabCorp, Sonora Quest or Patient's Choice.

03

We go through the results together

At a follow-up visit, we go through your results in plain language. You hear what we think is going on, what we would do about it, and what happens if you wait.

04

We start small and fine-tune from there

Your doctor starts with small changes, not everything at once. Follow-up labs show how your body responds. Your thyroid levels and how you feel tell us what to change next.

What we look at

The Hashimoto's testing we run at Onyx

Your doctor chooses your testing from your symptoms and your history, not from a fixed list. That starts with what you tell us, and we believe you. Testing is there to show whether the cause is autoimmune, and to find the things underneath it that a routine screen never covers.

Thyroid antibodies

TPO and thyroglobulin antibodies show whether the cause is autoimmune, something TSH alone cannot tell you. After that, your thyroid is followed with TSH and free T4, because antibody levels are not a reliable guide to treatment.

A full thyroid panel

TSH along with free T3, free T4 and reverse T3, read together rather than one at a time, so your doctor can see the whole thyroid picture instead of a single screening number.

Nutrient levels

Vitamin D and iron, and in some cases selenium and zinc. Each plays a role in thyroid function and immune health, and when a level comes back low, it can often be corrected.

Gut health

Since gut health and autoimmune activity are so closely tied, this gets looked at directly rather than assumed. That may mean a stool test, depending on what your symptoms point to.

Food sensitivities

When your symptoms and your history make it relevant. Not as a routine add-on, and not as a way to sell you a longer list of foods to avoid.

Your history, read next to the numbers

Your results inform the assessment. Your doctor reads them next to your symptoms and your history, and you decide together where to go from there. A number on its own never decides your plan.

Hashimoto's treatment

How we treat Hashimoto's

Your treatment plan is based on your symptoms, health history, and test results. Depending on what we find, your plan may include thyroid hormone support, gut health care, targeted nutrients, nutrition changes, and support for sleep and stress. We explain why each part of your plan may help, so you know what we are doing and why. We will also tell you when your symptoms may be coming from something other than your thyroid.

Food changes that fit your life

Food changes are also based on your needs. We do not put everyone on the same strict diet or ask you to cut out foods without a reason. If we find a food or other factor that may be affecting you, we will help you make changes that are realistic and easy to follow.

What to expect over time

Some patients start to notice improvement within the first 4 to 12 weeks, although some changes take longer. Your thyroid levels and your symptoms are what we follow, rather than judging your results from one follow-up visit.

Acupuncture and IV nutrient therapy are also available when they fit your care plan.

Why Onyx

Why Gilbert and East Valley patients choose Onyx for Hashimoto's care

If you have seen multiple doctors, been told your labs are normal, and still do not feel well, you are not alone. At Onyx, we start by listening to your symptoms, history, and concerns before deciding what needs to be evaluated. When appropriate, we may look beyond a standard thyroid panel, including additional markers that can help provide a fuller picture. And if your symptoms point to something other than your thyroid, we will help you explore that too.
  • A 75-minute first visitLong enough to take the whole history, not just the symptom you booked for.
  • Antibodies are part of the workupTested because they show whether the cause is autoimmune, not left as an extra you have to know to request.

Frequently Asked Questions

Hashimoto’s Disease

Yes. Thyroid antibodies can be raised for years before TSH moves out of range, and a routine screen usually does not check antibodies at all. That is why antibody testing is part of the workup here rather than something you have to know to request. Finding it early changes what we watch for and what we do about it. The American Thyroid Association says antibody testing helps make the first diagnosis. TSH and free T4 are what show how your thyroid is working.

Hashimoto’s is a cause. Hypothyroidism is a description. Hashimoto’s is an autoimmune condition where your immune system attacks the thyroid, and over time that attack is the most common reason a thyroid becomes underactive in the United States. Hypothyroidism just means the thyroid is not making enough hormone, whatever the reason.

Stress, an illness, poor sleep and food are the ones patients name most often, and it is not always obvious which one set a particular flare off. Triggers are personal, so part of the work is tracking yours instead of working from a general list.

Not automatically. Some people with Hashimoto’s see their antibody levels settle on a gluten-free diet and some see no change, and the research is real but limited. Your doctor talks through whether it is worth trying in your case rather than handing you the strictest version of a diet you read about online. The NIH says researchers are still studying how diet and supplements affect Hashimoto’s. There is no specific guidance yet.

Symptoms can improve a lot and antibody levels can come back into a normal range, and that is what remission means. It is not the same as the condition being gone, it is not permanent, and it does not happen for everyone. Your doctor talks through what is realistic in your situation instead of promising you an outcome. An NIH review reports that up to a quarter of people who are hypothyroid may return to normal thyroid function. That can take several years.

No referral is needed. If you have recent thyroid labs, bring them and your doctor will review them. Most patients still need a wider panel than routine screening covers, antibodies included, so expect some testing either way.

Lab work is often billable to your insurance even though our visits are not, and coverage depends on your plan and your diagnosis codes. Onyx is primarily a self-pay clinic for appointments. Many patients use HSA or FSA funds, and the clinic provides documentation you can submit for out-of-network consideration. More on insurance.

Yes. Consultations, lab review and follow-up are all available by telemedicine, and we see patients all over the country. Your lab draws are done at a laboratory near you. Anything that has to happen in person is done at the Gilbert office.
