THE SHORT ANSWER
What Is Hormone Replacement Therapy?
Hormone replacement therapy, or HRT, uses prescribed hormones to address symptoms associated with declining or insufficient hormone levels, most often estrogen, progesterone and testosterone. Those symptoms commonly include hot flashes, broken sleep, low energy, mood changes and low libido.
Onyx is a naturopathic clinic, so hormone therapy is one tool inside a fuller workup. Before anyone writes a prescription, your doctor reviews your history alongside appropriate testing, because thyroid, metabolic, nutritional and other factors can contribute to similar symptoms.
IS THIS YOU
Who May Benefit From HRT?
HRT is not right for everyone. Your medical history, your symptoms and appropriate testing help determine whether hormone therapy is appropriate for you.
Women in Perimenopause
Levels can swing sharply for years before periods stop, and the symptoms rarely arrive one at a time.
Women in Menopause
Once periods have stopped for a year, the question is usually which route suits you rather than whether something is happening.
Men With Low Testosterone
Testosterone declines gradually, so men often adjust to feeling flat rather than noticing a change.
Adults With Hormone-Related Symptoms
When symptoms warrant a closer look at hormone levels, alongside the other factors that can produce them.
SOUND FAMILIAR?
Symptoms of Hormonal Imbalance
Hormone symptoms are easy to explain away one at a time. It is the pattern that matters, and the fact that it has not lifted on its own.
Exhausted no matter how long you sleep?
Has your sex drive quietly disappeared?
Weight settling around your middle despite no change in diet?
Forgetting words, names, or why you walked into the room?
Hot flashes or night sweats breaking up your nights?
Anxious, irritable, or just not like yourself?
Are your periods irregular, heavy or painful?
Has your strength or recovery from workouts dropped off?
These symptoms can have many possible causes, so testing and clinical evaluation are important before attributing them to hormone changes.
Types of Hormone Replacement Therapy
Hormone replacement therapy is not one prescription. Which hormone, or which combination, is appropriate depends on your symptoms, your history and what your testing shows.
Estrogen therapy
Most often estradiol, the main estrogen of the reproductive years. Commonly considered where symptoms follow the menopausal transition.
Progesterone therapy
Balances estrogen's effect on the uterine lining. Whether it belongs in your plan depends on your history and your results.
Testosterone therapy
Relevant for women as well as men. Total and free testosterone are both read before it is considered.
Combination therapy
More than one hormone together, where your results and symptoms point that way rather than as a starting assumption.
HRT Delivery Options
Most patients arrive already leaning one way, usually because of something they read or a friend's experience. Every route below is legitimate; what differs is how often you think about it and how quickly the strength can be changed.
Creams
Applied dailyApplied at home on a daily rhythm and absorbed through the skin. The easiest route to adjust, which often suits the months while a strength is still being settled.
Trade-off: Easy to miss a day, and care is needed around skin contact.
Gels
Applied dailyAlso applied to the skin daily. Creams and gels are grouped together clinically; what differs is the preparation rather than how the hormone gets in.
Trade-off: Easy to miss a day, and care is needed around skin contact.
Injections
On a set scheduleMost often used for testosterone, and usually self-administered at home once you have been shown how. Responds quickly while a plan is still being settled.
Trade-off: A routine to keep, and levels move between doses.
Patches
On a set scheduleWorn on the skin and changed on a set schedule, without a daily step to remember.
Trade-off: A routine to keep, and levels move between doses.
Oral medication
Taken by mouthMost often used for progesterone, and usually taken at night. How a hormone is metabolised depends partly on whether it is swallowed or absorbed another way.
Trade-off: The route itself changes how the hormone is processed.
Pellets
Placed in officeInserted under the skin in a short visit, then released steadily for months. The lowest-maintenance route, with nothing to remember day to day.
Trade-off: The strength cannot be changed until the next insertion.
Your doctor recommends a route based on the hormone you need, your results and how quickly the strength may need to change. You are not locked into the first one.
Bioidentical vs. Traditional HRT
The terms HRT and bioidentical HRT are often used interchangeably, but they do not mean exactly the same thing. HRT is the broader term for hormone replacement therapy, while "bioidentical" describes the molecular structure of the hormone being used.
| Question | Bioidentical HRT (BHRT) | Hormone replacement therapy (HRT) |
|---|---|---|
| What does it mean? | Uses hormones with the same molecular structure as hormones naturally produced by the body. | A broad term for treatment that replaces hormones your body is no longer producing enough of. |
| What hormones can be used? | May include bioidentical estradiol, progesterone, or testosterone. | May include bioidentical or non-bioidentical hormones, depending on the treatment. |
| Where does it come from? | Can be available as an FDA-approved medication or a compounded preparation. Compounded preparations are not FDA-approved as finished products. | Can also be available in FDA-approved or compounded preparations, depending on the medication. |
| How is treatment chosen? | Based on your symptoms, medical history, and clinical evaluation. | Also based on your symptoms, medical history, and clinical evaluation. |
When people say traditional HRT they usually mean a preparation that is not bioidentical. The table uses HRT in its broader sense, which is how the term is used clinically, so the two columns overlap rather than sitting opposite each other.
What to Expect During HRT Treatment
Hormone therapy starts with an evaluation, not a prescription. Your provider reviews your symptoms, health history, and appropriate lab results before determining whether HRT is right for you. From there, your treatment is monitored and adjusted as needed.
- 01
Initial Consultation
Discuss your symptoms, health history, medications, and treatment goals with your provider.
- 02
Comprehensive Evaluation
Complete appropriate lab testing to assess hormone levels and other factors that may contribute to your symptoms.
- 03
Personalized Treatment Plan
If HRT is appropriate, your provider recommends a treatment approach based on your results, health history, and individual needs.
- 04
Follow-Up & Monitoring
Your response to treatment is monitored over time. Follow-up visits and testing help your provider determine whether adjustments are needed.
WHAT PATIENTS ARE HOPING FOR
Benefits of HRT
What hormone therapy can do depends on which hormone is low, how long it has been low, and what else is going on underneath. These are potential benefits, not promises, and none of them is a treatment guarantee.
Fewer hot flashes and night sweats
The symptom hormone therapy has the longest track record for, and often the one that shifts earliest.
Sleep that holds through the night
Waking at a consistent hour is a different problem from struggling to fall asleep, and the two point at different drivers.
Steadier mood
Irritability or anxiety that arrived without a reason, or that tracks your cycle, is worth reading against your hormones rather than assuming it is stress.
Energy through the afternoon
Fatigue has a wide range of drivers, which is why thyroid and cortisol are read alongside sex hormones before anything is attributed to one of them.
Libido and drive
Total and free testosterone matter here for women as well as men, and a long stress load suppresses both.
Support for bone density
Estrogen therapy is recognised for reducing the bone loss that follows menopause. Whether it is appropriate for you depends on your age and your other risk factors.
HRT Monitoring & Follow-Up
Hormone therapy is not a set-and-forget prescription. Your response is reviewed over time, and your plan is adjusted as your needs change.
Follow-up visits
Your provider reviews how you are responding, what has changed and what has not. Follow-up is part of the plan rather than something you have to ask for.
Retesting, when appropriate
When indicated, repeat testing helps your provider see how your levels have responded alongside how you are actually feeling.
Adjusting your treatment
The hormone, the dose or the delivery method may be adjusted. Needing an adjustment is common and is not a sign that something has gone wrong.
Continuing, changing or stopping
Your provider determines whether treatment should be continued, adjusted or changed. Hormone therapy is not necessarily permanent.
Why Choose Onyx for HRT?
If you have been told your labs are normal and sent home, we start by listening, then we test more broadly, because the answer often sits in markers a standard panel does not cover. And if your symptoms turn out to be driven by something other than your hormones, we will tell you that too.

Frequently Asked Questions
Hormone Replacement Therapy
How do I know if my hormones are imbalanced?
The only way to know is to test, because the common symptoms of hormone imbalance overlap with several other conditions. Fatigue, weight changes, low libido, poor sleep, mood changes and brain fog can all point to hormones, and they can also point to thyroid, iron, blood sugar or sleep problems. That overlap is exactly why we test before we treat rather than working from symptoms alone.
How do I get all my hormone levels checked?
At Onyx, your naturopathic doctor orders a comprehensive panel at your first visit and reviews every result with you at a follow-up appointment. You do not need a referral. Most patients complete their lab work locally in Gilbert, and we go through the full picture together rather than sending you a portal result to interpret on your own.
Will insurance cover hormone testing?
Lab work is often billable to your insurance even though our visits are not, and coverage depends on your specific plan and diagnosis codes. Onyx is primarily a cash-pay clinic for appointments. Many patients use HSA or FSA funds, and we can provide documentation you can submit to your insurer for out-of-network consideration.
How much does hormone treatment cost at Onyx?
Cost depends on your testing and your treatment plan, and we go over the full picture with you before anything begins. A deposit secures your appointment and applies toward that visit. We do not run free consultations, because your first appointment is a real clinical evaluation rather than a sales conversation.
Is hormone replacement therapy safe?
Hormone therapy is considered appropriate for many patients when it is prescribed and monitored by a licensed clinician, and suitability depends on your health history, your labs and your risk factors. That assessment is part of your first visit and your lab review. We start conservatively, we recheck your levels, and we adjust based on what your follow-up testing shows.
How long until I start to feel different?
This varies considerably from person to person, and it depends on what is driving your symptoms and which approach your plan uses. Some people notice changes early on, others take longer, and some need their plan adjusted after the first recheck. We schedule follow-up testing specifically so we are tracking your response rather than guessing at it.
What kind of doctor treats a hormone imbalance?
A naturopathic doctor, an endocrinologist or a hormone-focused physician can all evaluate a hormone imbalance. At Onyx you see a licensed naturopathic doctor (NMD), who is trained to look at how your systems interact and typically spends considerably longer with you than a standard primary care appointment allows. Your primary care doctor is often working inside a fifteen-minute visit and a panel your insurance will authorize. We order broader testing, interpret it with you, and treat hormones alongside metabolism, nutrition and stress rather than as a separate specialty.
