Sound familiar

Signs you might be in perimenopause

You do not need every one of these. Most women get a handful at once, and they usually show up in ways that do not look connected at first.

Periods that are heavier, lighter, closer together, or further apart

Hot flashes, or night sweats that wake you up

Trouble falling asleep, or waking at the same hour every night

Mood swings, new anxiety, or feeling low without a reason

Brain fog, losing words, or trouble holding your focus

Low energy that a full night of sleep does not fix

Weight gain around your middle with nothing else changed

Lower sex drive, vaginal dryness, thinning hair, or new headaches

If a few of these sound familiar, you are not imagining it. Most of the women who come to us have been told each symptom is its own separate problem. Read together, they usually point somewhere.

Perimenopause

What Is Perimenopause?

Perimenopause is the stage when your body begins moving toward menopause. During this time, hormone levels change, which can affect your periods, sleep, mood, energy, weight, and other parts of your health.

It usually begins in your 40s, but it can start earlier. Symptoms can come and go, and they may change over time.

Perimenopause is the transition. Menopause is the point when that transition is over. Perimenopause can last for several years and ends when you have gone 12 months without a period. At that point, you have reached menopause.

If you are still having periods, even if they have become irregular, you may be in perimenopause. If you have gone 12 months without a period, you are considered to have reached menopause.

Perimenopause lab testing

Told your labs were normal? Here is why that happens

Perimenopause can be difficult to identify because your hormone levels can change from week to week. A single hormone test may look normal even when you are having real symptoms. That is why we do not rely on one test result to explain how you feel.

Your doctor looks at the full picture, including your symptoms, menstrual cycle, health history, and any changes you have noticed. When testing is needed, we may also look at your hormones, thyroid, nutrient levels, and other factors that can cause similar symptoms or make them worse.

Your results are then reviewed alongside your symptoms and history. This gives you a clearer picture of what may be happening and helps your doctor recommend treatment based on your individual needs.

Not sure whether this is perimenopause? That is what a first visit is for.

How we start

What your first visit for perimenopause looks like

Every patient starts the same way. What changes is where your history and your results take the plan.

01

A 75-minute first visit

You get the whole history. What changed, when it changed, what you have already tried, and what you want back. You will finish your sentences.

02

The testing your picture calls for

Your doctor chooses the panel from your symptoms and your history rather than a fixed list. Draws are done locally through LabCorp, Sonora Quest or Patient's Choice.

03

We go through the results together

You come back for a follow-up visit and we read them with you, in plain language. You hear what we think is going on, what we would do about it, and what happens if you would rather wait.

04

We decide together, then check back

Your doctor recommends and you decide. Then we follow up, see how you are actually feeling, and adjust from there.

Perimenopause treatment

How we treat perimenopause at Onyx

Once we know what is actually going on, we build the plan around it. Depending on what we find, that can include bioidentical hormone therapy, which uses hormones with the same molecular structure as the ones your body makes and comes as creams, patches or pellets. It can also include herbal medicine aimed at the symptoms bothering you most, targeted nutrition for the gaps your testing turns up, and changes to sleep, movement and stress that fit your actual life.

Hormone therapy is not the only path and it is not right for everyone. Some patients do well with nutrition and lifestyle changes on their own. Some have something else going on that hormones would not touch. We will tell you that plainly, including when we think the answer is not a prescription from us. Some patients notice a difference in the first 4 to 12 weeks, and others take longer.

More on how the forms differ is on our bioidentical hormone therapy page.

Why Onyx

Why women across Gilbert and the East Valley choose Onyx for perimenopause care

Most of the women who reach us have already been somewhere. They were told their labs looked normal and sent home, or handed something for the one symptom they mentioned out loud. What changes here is how long the first visit is, how wide the first workup goes, and the fact that somebody keeps adjusting it afterwards. And if your symptoms turn out to be driven by something other than your hormones, you will hear that instead of a prescription.

  • A 75-minute first visitLong enough to take the whole history, not just the symptom you booked for.
  • Testing chosen for your pictureOrdered because your symptoms and your history point at it, not from a fixed panel.
  • Results read with youIn plain language at a follow-up visit, not sent to a portal without comment.
  • We will tell you when hormones are not the answerIncluding when we think you do not need a prescription from us.
  • Follow-up built into the planRetesting and adjustment are part of the arrangement, not an add-on you have to ask for.
  • Three licensed naturopathic doctorsOne Gilbert clinic, in person or by telemedicine nationwide.

The naturopathic doctors behind your perimenopause plan

Dr. Stephanie Nichols, NMD

Onyx's founder, working mainly in women's hormones, thyroid health and the conditions that overlap with both. On a perimenopause case the first thing she wants is the cycle history, because the pattern over the last two years usually says more than any single result.

Founder · Naturopathic Physician

Dr. Kasey Nichols, NMD

Connects hormones, digestion and mindset. He is the one most likely to catch the sleep, gut or stress piece sitting underneath a hormone complaint.

Co-Founder · Naturopathic Physician

Dr. Chelsea Deveson, NMD

Focused on the root causes of chronic disease, including thyroid conditions, autoimmunity, adrenal fatigue and burnout, PCOS (now officially PMOS), perimenopause and digestive issues.

Frequently Asked Questions

Perimenopause Treatment

Onyx is primarily self-pay for appointments. Lab work is often billable to your insurance even though visits are not, and that depends on your plan and your diagnosis codes. Many patients use HSA or FSA funds, and we can provide documentation you can submit for out-of-network consideration. What you spend depends on your testing and your plan, and we go through it with you before anything is ordered. A deposit secures your appointment and applies toward that visit.

Probably not. Most women notice changes in their early to mid forties, and it is common for it to start in the late thirties. Age on its own does not rule it in or out. What matters more is the pattern in your cycles over the last year or two, which is one of the first things your doctor will ask you about.

Not on its own. In perimenopause your hormone levels move week to week, so one draw catches one moment and can easily read as normal. Perimenopause is diagnosed from your cycle history and your symptoms, which is why we start by listening. We still test, because testing is how we find the thyroid, iron, blood sugar or nutrient problems that cause the same symptoms or make them worse.

No referral is needed. If you have recent lab work, bring it and your doctor will review it. Most patients still need a wider panel than routine screening covers, so expect some testing either way.

No. Hormone therapy is one option and it is not right for everyone. Some patients do well with nutrition, herbal medicine and lifestyle changes. Your doctor will walk you through what fits your situation and what the trade-offs are, and you decide.

For most healthy women in perimenopause, yes. The Menopause Society says the benefits generally outweigh the risks for healthy women under 60. At Onyx we use bioidentical hormones. They have the same structure as the hormones your body makes. Your doctor starts you on a low dose and only raises it if you need it. Follow-up visits and testing help catch side effects early, so your plan can change. Your health history, age and risk factors still matter. Your licensed naturopathic doctor (NMD) goes over them at your first visit.

This varies from person to person, and it depends on what is driving your symptoms and what your plan uses. Some patients notice a difference in the first 4 to 12 weeks, and others take longer. We schedule follow-up specifically so we are tracking your response rather than guessing at it.

Yes. Consultations, lab review and follow-up are all available by telemedicine, and we see patients all over the country. Your lab draws are done at a laboratory near you. Anything that has to happen in person, such as pellet insertion, is done at the Gilbert office.
