Tirzepatide is a prescription medication used for weight management, given as a once-weekly injection under the skin. It works by activating two hormone receptors involved in appetite, food intake and blood sugar regulation: GLP-1 and GIP.

Who is a candidate for tirzepatide?

There is no single timeline that applies to everyone. Some patients notice changes in appetite within the first few weeks, while meaningful weight loss can take longer. Your response depends on factors including your starting weight, dose, nutrition, activity level, health history and how your body tolerates treatment.

Dosing

Tirzepatide dosage and titration

Tirzepatide is started at a low dose and increased gradually to help your body adjust and reduce the likelihood of side effects. The schedule below reflects the FDA-approved starting and titration framework for Zepbound. Your doctor determines your actual dosing schedule based on your response, tolerability and clinical circumstances.

Weeks 1 to 4

2.5 mg weekly

The recommended starting dose. It is there to let your body adjust to the medicine, and is not intended to drive weight loss on its own.

Weeks 5 to 8

5 mg weekly

After at least four weeks at 2.5 mg, the dose may step up to 5 mg weekly if you are tolerating it well. Some patients stay at this dose.

Month 3 onward

Dose increases

If more is needed and the medicine is well tolerated, the dose can rise in 2.5 mg steps, no sooner than every four weeks at the current dose.

Maintenance

The label's maintenance doses are 5 mg, 10 mg and 15 mg weekly. At Onyx many patients maintain below that, commonly 1 mg to 3 mg weekly.

Low-dose and microdosing

Not every patient is working toward the highest tolerated dose. Onyx also prescribes tirzepatide at low doses, often starting at 1 mg or 1.5 mg weekly, for long term maintenance and for patients using it to support inflammation, PCOS or PMOS, an autoimmune condition or endometriosis.

These doses sit below the titration schedule on the label and are prescribed off label, adjusted to your response rather than to a fixed protocol. Your doctor will go through what that means for you.

Safety

What are the side effects of tirzepatide?

Most side effects are digestive. They tend to appear in the days after a dose increase and ease as your body adjusts. Many patients have only mild side effects, or none at all, which we think reflects dosing to the individual rather than running everyone through the same protocol. Patients who found semaglutide difficult to tolerate often report an easier time on tirzepatide, though this varies and cannot be predicted in advance.

Less common risks

Pancreatitis, gallbladder problems, kidney injury from dehydration and changes in vision in people with diabetic retinopathy are uncommon but possible. Your doctor reviews your history for these before prescribing.

Tirzepatide on its own rarely causes low blood sugar. The risk is higher if you also take insulin or a sulfonylurea, and those doses may need to be reduced when tirzepatide is started. Tell your doctor about every medication you take.

Who should not take tirzepatide

Tirzepatide is not prescribed if

  • You or a family member has had medullary thyroid carcinoma, or you have Multiple Endocrine Neoplasia syndrome type 2.
  • You have had a serious allergic reaction to tirzepatide or to any of its ingredients.
  • You are pregnant. Tirzepatide is not recommended during pregnancy, and treatment is stopped once a pregnancy is recognized.
  • You are already taking another tirzepatide product or another GLP-1 medication. These are not combined.

Tell your doctor before starting

  • If you take an oral hormonal contraceptive. The labeling advises switching to a non-oral method, or adding a barrier method, for four weeks after starting tirzepatide and for four weeks after each dose increase.
  • If you are breastfeeding or planning a pregnancy.
  • If you have a history of pancreatitis, gallbladder disease, severe digestive problems or delayed stomach emptying.
  • If you have kidney problems or diabetic retinopathy.
  • About every medication and supplement you take, and about any planned surgery or procedure involving anesthesia.

What happens when you stop tirzepatide?

Appetite usually returns when the medication stops, and some weight regain is common. In the SURMOUNT-4 trial, participants who stopped tirzepatide after 36 weeks regained part of the weight over the following year, while those who continued lost more.

That makes stopping worth planning in advance. Depending on your situation that may mean a lower maintenance dose, a plan built around nutrition and strength training, or staying on treatment longer term. Your doctor talks this through with you before you start.

Onyx is a cash-pay clinic for office visits. Naturopathic doctors are not in insurance networks, so visits cannot be billed to insurance, though HSA and FSA cards are accepted.
